Insurance Coverage for Dr. Miwa's Psychological Services

Dr. Miwa is not an “in-network provider” for managed care. However, a monthly “Superbill” can be provided for clients who have PPO insurance to allow them to submit an insurance claim for reimbursement.

HELPFUL INFORMATION ABOUT A SUPERBILL:

If you have PPO insurance coverage, here are a few general guidelines on how you will receive coverage for my services from your insurance provider.

My practice is NOT "in-network” for any PPO insurance providers.  However, that doesn’t mean your insurance provider won’t cover you for some or all of my fess; it just means that you will need to submit a claim yourself directly to your insurance provider.

For clients with PPO insurance, we will provide you with what is known as a “Superbill.” Your insurance provider will understand this term. A Superbill is a statement of services provided to you, detailing the individual session dates, the medical billing code and its description, and the amount you were charged for each session. We will provide a Superbill at the end of each calendar month. You will submit the Superbill to your insurance provider when you make a claim for reimbursement.

Just as with any medical costs you are planning to  incur and claim from your insurance provider, you are recommended to contact your insurance provider ahead of time to confirm coverage amounts.

Check the following with your insurance company:

1)Will they cover you for services from a provider who is “out of network.” (Theoretically, PPO insurance should cover you “out of network,” but have this confirmed).

2) If you will be covered, will there be any deductibles you have to meet first before receiving reimbursement.

3) Ask how much reimbursement you will be covered for a 50-minute session. From my experience with a variety of PPO plans, the reimbursement rates vary from 20% to 90%, depending on your company and plan.

After contacting your insurance company and establishing how much coverage you will receive, if my full fee is still going to be too high for your financial circumstances, please note that I work on a sliding-scale of fees in such cases for potential clients who seem likely to benefit from my services. After you have talked to your insurance company and determined your coverage, please do not hesitate to discuss your financial concerns with me before or during your intake appointment.

Please don’t hesitate to contact me with any questions you may still have.

Yours,

Dr. Miwa
